Job Title: Band 6 Occupational Therapist – Intermediate Care & Community (I&CC)

Salary: £26 per hour (Hourly)
Duration: 3 months
A fantastic opportunity awaits! We are on the lookout for a Band 6 Occupational Therapist specialising in Intermediate Care & Community for a locum position in the vibrant area of Swindon. This role is perfect if you are looking to step out of your comfort zone and make a real difference in the community over the next three months. You will be part of an enthusiastic team dedicated to providing excellent care.
What you will do:
* Conduct comprehensive patient assessments to create personalized care plans that promote health and well-being within the community.
* Collaborate with multidisciplinary teams to facilitate seamless transitions and interventions that prevent hospital admissions and support effective discharges.
* Manage your caseload independently and ensure high-quality, patient-centred care.
* Implement evidence-based practice and treatment strategies, adjusting plans to meet evolving patient needs.
* Engage with families, carers, and community services to ensure that holistic support is in place for patients.
* Participate in training and development sessions to lead and support junior team members.
* Reporting to - Front Door Team Lead.
* The staff member will be required to work their contracted hours over 7 days a week.
Requirements:
* State Registered HCPC.
* Diploma/Degree in Physiotherapy or Occupational Therapy.
* Experience in complex discharge planning within the NHS.
* Evidence of postgraduate learning relevant to clinical area.
* Working as part of a MDT and joint problem solving.
* Supervision and training of others.
* Broad clinical experience e.g. surgical, ED, neurology, medical, orthopaedic, hand injuries, rehabilitation.
* Experience of providing a broad range of interventions for a complex caseload.
* Use of goal setting and outcome measures.
* Experience of providing supervision for junior staff or assistants.
* Lead for student supervision.
* Carrying out and receiving appraisal.
* Experience in and an understanding of clinical governance e.g. audit, risk management, education.
