Position Functions:
Assist and support Registered Nurses and other members of the Interdisciplinary Resident Care Team on the assigned Resident Home Area (RHA). Responsibilities include planning, implementing, evaluating our resident-centred care model, humanizing the environment, and encouraging involvement of staff, residents, families, and visitors in care components.
Responsibilities
Under the direction of registered nurses, the registered practical nurse shall:
* Plan, implement, delegate, supervise, and evaluate care provided by non-regulated staff in assigned RHAs, in accordance with legislation and regulations of the Fixing Long Term Care Act, 2021, and standards of the College of Nurses of Ontario.
Ensure and promote resident-centred care
Provide resident-centred and personal care services that meet residents' needs, including:
* Supporting and promoting Residents’ Bill of Rights.
* Fostering care that reflects residents’ values and preferences.
* Meeting physical, psychosocial, social, spiritual, cultural, and ethnic needs through interdisciplinary teamwork.
* Ensuring compliance with all applicable legislation and policies.
* Maintaining a wellness and restorative approach.
* Creating opportunities for residents to make choices and maintain control.
* Providing a comfortable, safe, and dignified environment.
* Facilitating relationships among residents, families, staff, and the community.
Provide Leadership for Nursing and Personal Support Care
* Demonstrate commitment to the Home’s Mission, Vision, and Values.
* Manage medication, treatments, and documentation as per regulations.
* Participate in team meetings, conferences, and quality assurance programs.
* Mentor non-regulated staff and support their professional development.
* Participate in strategic planning and research initiatives.
* Act as a team leader in resident admission, transfer, and discharge processes.
Support Effective Human Resources Practices
* Monitor staff compliance and assist in staff development and performance appraisals.
* Role model professional development and participate in home committees.
Ensure Occupational Health and Safety
* Conduct safety rounds and report unsafe conditions.
* Ensure staff competency and safe practices.
Provide Effective Use of Material Resources
* Oversee supplies and equipment, provide education on their use, and promote continuity of care.
Maintain Recording Systems
* Ensure documentation compliance and confidentiality.
Maintain Registration with the College of Nurses of Ontario
* Maintain current registration and submit proof annually.
Implement Infection Prevention and Control Measures
* Practice proper hand hygiene, universal precautions, and risk assessments.
Other Duties
* Perform additional tasks as assigned by the Director of Resident Care or designate.
Qualifications:
* Current certificate of competence from the College of Nurses of Ontario.
* Valid CPR and First Aid certificates.
* Desirable: Certification in Gerontology, membership with GNA and RNAO.
* Minimum one-year experience in geriatric, rehabilitation, or long-term care preferred.
* Willing to work various shifts and weekends.
* Strong judgment, leadership, supervisory, and communication skills.
* Proficiency in English; Cantonese/Mandarin skills are assets.
* Computer literacy.
