Job overview
An exciting and rare opportunity has arisen for a Band 6 Prolonged Disorder of Consciousness Clinical Nurse Specialist (PDOC) to join the Specialist Neurorehabilitation PDOC team at Llandough hospital within Cardiff and Vale University Health Board.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The Role of the PDOC Clinical Nurse Specialist (CNS) will be integral to the complex care transition and ongoing ward-based nursing management from an acute care environment into a specialist PDOC bed and assessment environment.
The CNS will provide valuable and personalized support to patients and their families. They will implement and support ward staff to provide a tailored PDOC nursing care plan and liaise regularly with the MDT and family to inform them of any clinical/nursing developments.
The CNS will have dedicated time to implement and support best practice when working with PDOC patients on the Neuro Rehabilitation Unit (NRU).

Detailed job description and main responsibilities
Clinical and Patient Care Responsibilities not limited to:
* To work as an autonomous practitioner
* To provide up to date evidence-based care.
* To provide clinical advice to all members of the interdisciplinary team, patients, families, carers and other agencies.
* To utilise nursing skills and knowledge to undertake specialist assessments, critically analysing complex information to make informed decisions regarding appropriate treatment/intervention to ensure the delivery of highly specialist care.
* To be responsible and accountable for legible and contemporaneous records.
* To evaluate and document care and treatment and outcomes
* To assess patients 24-hour care program, including, behaviour charts, sleep/wake and nursing intervention.
* To have a positive influential role and impact on care across the clinical area and work closely with the MDT.
* To understand the National guidelines, provide advice for end-of-life care in patients with prolonged disorders of consciousness (PDOC), work alongside the Band 7 and the palliative care team to support End of life.
* There will be an essential requirement to accompany the outreach team into the other Welsh Health Boards within the region.

Qualifications
* Registered Nurse (Part 1) Current NMC Registration
* Degree Pertinent postgraduate qualification, or working towards
Experience
* Knowledge of legislation pertinent to role (PDOC)
* Proven and relevant experience at Band 6
* Able to demonstrate sound knowledge of NMC Code (2018)
* Proven post-registration experience and evidence of coordinating and managing a team of staff.
* Experience of working in a multi- professional environment
* Experience of supporting patients and families with life changing injuries.
Skills
* Able to demonstrate an awareness of professional issues and developments.
* Able to relate theory into practice through reflective skills
* Good knowledge of information governance/data protection requirements
* Able to demonstrate research based practice
* Ability to delegate and prioritise work. Able to manage time effectively
Special Knowledge
* Appropriate experience within specialty or able to demonstrate sound knowledge of relevant specialty
* Evidence of effectively implementing change in the clinical practice
