Job summary Pelham Medical Group is looking to recruit a Practice Nurse for 18 hours per week to enhance our Practice Nursing Team. We are a welcoming practice in Grimsby aiming to provide high quality care in a supportive environment. We are looking for a committed, hardworking nurse with the enthusiasm to learn new skills and adapt to changing circumstances. Experience in general practice is preferred with competencies in immunisation and vaccinations, wound care and diabetes (or a minimum of one chronic disease) management. Additional skills of cytology, family planning and management of additional long term conditions will be advantageous. The successful applicant will take a share of the workload across our modern and well equipped purpose built sites. The role will include chronic disease management, cytology, treating minor illnesses, dressings, family planning, immunisations, vaccinations and health promotion. Full details of this role can be found in the job description. Main duties of the job The post holder is responsible for the delivery of practice nursing care to the practice population. They will deliver care within the boundaries of their role, focusing upon chronic disease management, supporting GPs in care planning and monitoring of patients at risk of unplanned hospital admissions, supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. You will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership to other staff as required.
