Social Worker I Leaving Care I Middlesbrough Council
Reed is currently looking to recruit an experienced Children’s Social Worker to join Middlesbrough Councils Leaving Care Team.
Fantastic payrate of £38 per hour!
Responsibilities:
* Develop and implement personalized pathway plans for each young person, outlining their goals and the steps needed to achieve them.
* Offer practical and emotional support, advocating for young people’s needs in areas such as education, employment, housing and health.
* Provide immediate support during crises, such as homelessness or mental health issues, ensuring young people have access to necessary services.
* Work closely with other professionals, including educators, healthcare providers, and housing officers, to coordinate comprehensive support.
* Regularly review and update pathway plans, ensuring they remain relevant and effective as young people’s needs and circumstances change.
Requirements of this role:
* Qualified Social Worker registered with Social Work England.
* Experience in pathway planning and working with young people.
* Strong crises intervention skills and the ability to provide immediate support.
* Excellent collaboration skills to work with a range of professionals.
* Commitment to continuous monitoring and reviewing of pathway plans.
