Detailed job plan can be provided on first contact This is a standard 10 session job plan (37.5hrs) (7DCC's & 3 SPA's) which will be subject to review three months after appointment and then no less frequently than annually This post at based at Cefn Coed Hospital responsible for the provision of care to the inpatients within the inpatient units at Gwelfor and Stepdown at Cefn Coed. Gwelfor is an eighteen bed male high dependancy rehabilitation/recovery unit for treatrment of patients with serious mental illnessess. Patients who are placed within the unit are usually either detained under the Mental Health Act 1983, Deprivation of Liberty Safeguards (MCA, DoLS) or are informal patients. The Step-Down service is a community rehabilitation unit which is comprised of two houses: Ty Gwanwyn which contains five beds and is staffed throughout the day and night, and Carreg Sarn, which contains three beds and has floating support throughout the day and staffed by night. The service receives referrals for patients that are at the stage in their recovery where they require support to build on their activities of daily living, assessing and improving their levels of personal independence, and testing of risk prior to moving back to the community. This is a replacement post joining a Speciality Doctor with the support of a Multidisciplinary Team inclusive of Occupational Therapy and Psycology. There is a General Practitioner who covers Gwelfor and Stepdown for one day week to see to the patients medical care. The medical team working on Gwelfor/step down work in partnership with a wider rehabilitation team including low secure services based at Taith Newydd (Glanrhyd Hospital, Bridgend) and a community Adult Forensic Community Team. The rehabilitation/recovery service on the unit is based on Recovery Model principles
