Care Coordinator

Meridian Business Support is currently recruiting for a Domiciliary Care Coordinator working for a national private healthcare provider. The brand provides community care to people living in their own homes in the Sudbury Area of Suffolk.
The Care Coordinator acts as the first point of contact for clients and staff, provides care in the community, and works closely with the Branch Manager and integrated care services to ensure a smooth delivery of services.
This is a full-time, 40-hour-per-week role with on-call responsibilities. Primarily community-based, the position will include office-based work as the branch expands.
Key duties and responsibilities
1. Develop and implement person-centred care plans, ensuring regular reviews and updates.
2. Shadow new carers, provide ongoing support and complete competency checks.
3. Provide care in the community.
4. Act as first point of call for clients and carers.
5. Report regularly to the Homecare Branch Manager.
6. Maintain dignity and standards of service users and their homes.
7. Work as part of an efficient team to ensure smooth running of the service.
8. Continue professional development through attending and contributing to training, meetings, and supervisions.
9. Comply with company Health & Safety policy and procedures.
10. Represent the company professionally and positively at all times.
11. Maintain confidentiality regarding all issues and information relating to service users.
12. Support service users' personal development regarding independence, skills, rights, and choices.
13. Efficiently manage staff rotas, ensuring timely distribution and accessibility.
14. Train, assess, and ensure that medication is administered and managed effectively and safely by staff.
15. Meet with service users to ensure that their care meets their needs as per their individual care plan.
16. Fulfil on-call responsibilities as required.
Skills and attributes
1. Two years' experience of working in a care setting with vulnerable people.
2. Willing to undertake NVQ or further training as needed.
3. Ability to show empathy, understanding, and experience of the needs of the service user.
4. Well-developed time management skills, with the ability to work under pressure.
5. Ability to make balanced, clear, and timely decisions.
6. Taking a pro-active approach to consulting and involving others in decision-making processes as appropriate.
7. Undertaking all mandatory training.
8. Undertaking training in specialist subjects.
9. Understanding of and clear commitment to equal opportunities.
10. Understanding of and clear commitment to health and safety practices.
