Action medication changes and follow-up care as advised in hospital letters or discharge summaries. Provide independent advice on medicines and escalate to other healthcare professionals when necessary. Attend clinical meetings to contribute to patient care planning and multidisciplinary discussions. Operational Support: Oversee repeat prescriptions and manage acute prescription requests. Ensure safe and efficient prescribing practices in line with local guidelines and formulary. Help reduce prescribing-related workloads for clinical and non-clinical staff. Quality Improvement: Develop and implement medicines management policies. Support compliance with CQC standards for safe and effective care.
