The post holder will be an experienced clinician in the field of specialist health care for people with a learning disability. Working in partnership with clients, carers, colleagues and other agencies, the post holder will ensure that excellent clinical standards are maintained within their own practice and within the team.
Based in Swindon, you will have the flexibility to work within a model that supports both on-site and hybrid working arrangements, depending on the nature of the role. Main Responsibility
* Assess, plan, implement and evaluate specialist interventions for people with learning disabilities that are person-centered and in consultation with the service user as part of their overall care plan. This will include specialist therapies and behavioral support, depending on the role of the clinician (for example, physiotherapy, OT, SALT or LD Nursing).
* Assess clients' understanding of treatment proposals and their capacity to give informed consent while working within the legal framework when the client lacks mental capacity to make the required decision.
* Assess risk and develop positive risk management plans, including crisis and contingency plans, for service users as part of a multi-disciplinary team.
* Apply specialist knowledge and skills to meet the needs of those with complex health needs, including challenging behavior.
* Take a lead role in the management of the general health and well-being of service users, acting as Care Coordinator as required.
* Safely discharge the service user at the end of your episode of care.
* Keep patient clinical records and statistics as per trust and professional guidelines with the use of information technology – ensuring timely, contemporaneous and accurate records.
* Write professional and accessible (Easyread) reports including clinical recommendations and guidance.

Qualified professionals registered with the NMC:
* Learning Disability Nurse
* Mental Health Nurse
Experience in the field of LD health and/or Autism.
At least one year post-registration experience.
