The Team's remit is to provide support across the area as needed based on demand, with a special focus on reviews of support plans, provider failure work, mental health hospital discharges, NHS Continuing healthcare assessments/reviews, reviews and annual reviews. This role provides a variety of case work in a supportive environment based at Riverside in Lowestoft and Carlton Court Hospital. You will use the Signs of Safety model to support assessment of the patient to ascertain their wishes and feelings and support them to achieve greater independence following their discharge from hospital. support safe and effective assessments/ Reviews for Adults living in the Waveney area. work as part of the Integrated team in partnership with our health colleagues complete Care Act Assessments to ensure compliance with the Care Act 2014 As a Senior Independence and Wellbeing Practitioner, manage complex case work and promote quality service provision Effective operational delivery Signs of Safety, and People at the Heart of Care to support timely interventions that maximise adult’s wellbeing and independence. You will work alongside health and community partners to support health care process and resources. Highlight opportunities for joint delivery and reduction in service cost with the health and care system You will explore the adults support systems and local networks to ensure the promotion of greater independence, choice, and control in line with the Care Act 2014 and People at the Heart of Care. You will need Good knowledge of Adult Social Care Services Proven ability to verbally explain and describe accurately. Excellent negotiating skills to liaise between customers, manager, and service providers. Proven experience and understanding of a person centred, enabling approach to working with people to maximise their independence and wellbeing.
