Our Health and Wellbeing Team is seeking to appoint a Peripatetic Coordinator, working across the Coastal, Moor2Sea and Newton Abbot site localities, with the option of working remotely from home on occasion.
You will be a central co-ordination point for the local multi-disciplinary teams including both health and social care.
You will liaise with health and social care colleagues in the local and wider community, including voluntary sector representatives to aid the safe and independent living of complex clients in the community.
Main duties of the job
1. Be a central co-ordination point for the local multi-disciplinary teams including both health and social care.
2. Facilitate and coordinate regular multi-disciplinary meetings and ensure follow through any actions relating to client discussion.
3. Liaise with health and social care colleagues in the local community including voluntary sector representatives.
4. Support health and social care team colleagues in the utilisation of a range of 'case finding tools.
5. Be responsible for a high-quality customer service function in recording contact information and supporting the initial prioritisation of contacts.
6. Maintain IT based information systems and take responsibility for the production of key performance data.

Our Health and Wellbeing Team is an integrated professional team which includes Co-ordinators, Social Care, Community Occupational Therapy, Community Physiotherapy, Intermediate Care, Community Nursing, Pharmacy, Dietetics, Support Workers and Volunteering in Health.
The team is proactive and has a forward-thinking core that promotes a positive and 'can do' attitude. The successful candidate will be dynamic and key in managing patients to remain in their own home.
Job responsibilities
Communication and Working Relationships
1. Communicate daily with colleagues from the Health and Social care team.
2. Contact clients on the phone to gather or impart basic information.
3. Liaise and form strong links with all members of the Multidisciplinary team and outside agencies.
4. Communicate medical and social information in a professional manner.

Customer Service (Social care)
1. Provide a client-centred approach ensuring respect, dignity, and understanding.

Information Technology and Administrative Duties
1. Possess excellent keyboard skills and ability to use multiple IT systems.
2. Ensure information governance requirements are adhered to.
3. Accurate inputting onto relevant IT systems.
Person Specification
Qualifications and training
* Good command of English language, verbal and written.
* Knowledge of office administration/customer services.
* Knowledge of Health and Social Care administration and processes.
* NVQ in Health and Social care.
Special Experience
* Effectively prioritise workload.
* Understanding of working with confidential information.
* Experience of working in an office environment.
* Knowledge/familiarity with medical terminology.
* Experience of working with healthcare professionals.
Knowledge and Skills
* Good knowledge of business admin procedures.
* Awareness of Care Act 2014 legislation.
* Effective keyboard/IT skills.
