Job summary We are North Cornwall Community Rehabilitation Team, covering a large rural area and providing physical rehabilitation to adults over 18 in their home environment. The aim of the service to help people to optimise and maintain their quality of life and independence. The service seeks to maximise each individual's potential by providing timely and goal orientated rehabilitation that is tailored to their needs. The team comprises of Occupational Therapists and Physiotherapists and Support Workers. The successful candidate will be working as part of the team providing primarily occupational therapy assessments and interventions, but will have scope for more generic working as appropriate and with the necessary training. This is a part time post working 23 hours over 3 days with the exact working pattern to be discussed in interview. This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements. Main duties of the job The purpose of the post is: o To undertake specific skilled occupational therapy support work (e.g. carrying out assessments using various treatment skills) to aid the rehabilitation of patients. To plan and implement interventions, using various treatment skills and to monitor progress and evaluate the outcome. o To encourage patients and carers in an active approach to regaining and maintaining personal independence. o To support the occupational therapists by carrying out timely reviews of rehab care plans. To actively participate in the discharge planning process from start to finish with support from Occupational Therapy colleagues and the MDT. Key Dimensions of the post are: o To practise autonomously, under the supervision of a qualified therapist reporting back patient progress and informing therapist of any problems o To hold own caseload of patients delegated by the qualified members of the team. o To practise as a lone worker.
