Main duties of the job
Thepost holder is an experienced pharmacist who, acting within their professionalcompetency, will provide clinical and pharmacological care to facilitate safe,effective, rational and cost effective prescribing. They will demonstrate safeclinical decision-making and expert care for patients within the generalpractice. They will work collaborativelywith the multi-disciplinary general practice team to meet the needs of patientsand to support the delivery of policy and procedures.
It will be necessary tohave knowledge of current Primary Care contracts, The Quality and OutcomesFramework, GMS contract, PCN contract and locally commissioned ICB contracts as this is avital element of the post.
It is envisaged thatpart of the role will include the mentoring of our Pharmacy Technician andPrescribing clerks.
Additionally thesuccessful candidate will be encouraged and supported to undertake any furthertraining required to ensure appropriate role development.

ProfessionalResponsibilities:
Assist GPs to help deliver effectiveimplementation of medicine management of patients
Contribute to the development of policies/ protocols / guidelines within the Practice.
Use of computer for monitoring, recording,recall and management of all patient information - (Practice uses EMIS Web)
Aid the Practice team with implementationof new initiatives in the delivery of medicine management services.
Formulate reports as required for thePractice Management and/or GP Partners.
Support the implementation ofevidence-based guidelines in conjunction with the Practice team.

Manage the repeatprescribing reauthorization process by reviewing medications reaching reviewdates; make necessary changes as an independent prescriber in collaborationwith the general practitioners

Medicines reconciliation
To reconcile medicinesfollowing discharge from hospitals, intermediate care and care homes, includingidentifying and rectifying unexplained changes and highlighting any concerns tothe GP who is the patients usual doctor.
Work with communitypharmacists to ensure patients receive the medication they need post discharge
Ensure a post dischargemedication care plan is in place including any dose titration or follow uptests
Medication review and Long Term Condition management
Undertake clinicalmedication reviews with patients with multi-morbidity and polypharmacy andimplement clinically indicated prescribing changes as an independentprescriber, working to protocols agreed with the partnership.
Visit housebound orvulnerable patients to undertake clinical medication reviews, attendingmultidisciplinary team meetings as required.
To participate in developingpharmacist-led clinics
To prescribe safe, effectiveand appropriate medication as defined by current legislative framework andwithin own scope of clinical competence.
When seeing patients formedication advice, provide safe, evidence-based, cost-effective, individualisedpatient care.
Offer a holistic service topatients and their families, developing where appropriate an on-going plan ofcare/support, with an emphasis on prevention and self-care.
Promote health andwell-being through the use of health promotion, health education, screening andtherapeutic communication skills.
Acute/minor illness and patient medicine queries
Manage caseload of patientswith common/minor/self-limiting ailments while working within the scope of practiceand limits of competence, referring to GP or healthcare colleagues whereappropriate
Provide patient facingappointments and telephone support for those with questions, queries andconcerns about their medicines
Answer all medicine relatedenquiries from GPs, other practice staff and other healthcare teams(Community nurses and pharmacy)
Quality improvement, training and development
Identify and act uponnational and local policy and guidance that effects patient safety through theuse of medicines including MRHA alerts, product withdrawal and emergingevidence from clinical trials
Work with practice teams andpatients to implement NICE guidelines
Increase quality and safetyof prescribing through mechanisms such as audit and PDSA cycles ICBprescribingadvisor implementing C&H prescribing guidelines
