You will provide care and support role for children and young people with a range of Learning Disabilities, Mental Health, Autism, Complex Needs and Challenging Behaviour. Establishing a good relationship using a range of communication tools with the people you support, you will be the key worker for a number of individuals. You will work to a person-centred support plan and recovery star, contributing to the team in meeting the outcomes and lifestyle choices of the people you support.
Support the individual(s) to keep safe and healthy in line with their Support Plan and Health Action Plan. Working with their lifestyle choices, personal care preferences and identified risk assessments. Key work with the Individual(s) reviewing and developing their support plan through understanding their lifestyle choices, circles of support, what is important to and for them, and supporting them to have more choice and control in their life. Advocate as part of the key working with, or on behalf of, individual(s) you support to get their voice and views heard ensuring that family and work colleagues are aware of any circumstances that have changed.
Work collaboratively with Individual(s), their families, colleagues and partner agencies to achieve the best possible outcomes for the people we support. Uphold quality in everything that you do to comply with relevant standards such as CQC and Topaz Care Policies and Procedures.
