Job summary The Community Urgent Care Team (CUCT) provides a comprehensive 24/7 health and social care triage service to patients and/or their carers/relatives with the aim of averting the need for hospital admission or early hospital discharge. Our service includes assessing complex and specialist needs to develop, implement and evaluate the appropriate programmes of care and wherever possible, support individuals to remain in a community setting whether it be their own home, a residential home or nursing home. We often continue to be involved with an individuals programme of care for up to 7 days from the point of referral. Main duties of the job Job title: Integrated Support Officer Department: Community Urgent Care Team Base: Westgate Park, Charlton Street, Grimsby, DN31 1SQ Hours: 1 x 24 hours per week Rota pattern: You will be required to work a rota pattern which includes weekends and Bank Holidays Permanent Salary Range 3 £23,807 pro rata per annum fixed salary point The Integrated Support Officer provides a short term crisis intervention service to individuals within their own home environment that could include personal care and any support that has been identified through the assessment process. You will work in partnership with nurses, physiotherapists, occupational therapists or care managers across the Intermediate Tier Service and undertake delegated duties after appropriate instruction in conjunction with a detailed support/outcome plan. You will need to be able to work well as a team as well as using your own initiative and you will need to have good communication and customer care skills. It is essential that you have 2 years experience of working in a health and social care setting and have knowledge of the principles of enablement and self-care principles.
