About the Role:
We are looking for an enthusiastic and dedicated Adult Social Worker to join the First North & West and First South teams, providing high-quality social work services to adults with varying support needs across a diverse geographic area. The ideal candidate will have experience working in adult social care, including assessments, care planning, and support delivery, and be passionate about making a difference in people’s lives. Key Responsibilities:
* Conduct thorough assessments to identify the needs of adults with care and support requirements, using the Care Act 2014 and Mental Capacity Act 2005 frameworks.
* Develop and implement person-centred care plans, ensuring that individuals are supported to live as independently as possible and are involved in decisions about their care.
* Review and update care plans regularly to ensure they remain appropriate to the person’s evolving needs.
* Provide advice, guidance, and support to adults, carers, and families, ensuring they have the information and resources needed to make informed decisions.
* Empower individuals to take control of their care and support, including signposting to community resources, and promoting independence where possible.
* Offer practical support in accessing housing, financial support, and other relevant services.
* Assess and manage risks associated with care and support needs, ensuring individuals’ safety while respecting their rights.
* Take appropriate action in line with safeguarding protocols, working to protect vulnerable adults from harm.
* Ensure that all assessments, care plans, and interventions are in accordance with the Safeguarding Adults Board policies and procedures.
* Work collaboratively with health professionals, housing, mental health services, and other relevant stakeholders to deliver a coordinated approach to adult care.
* Contribute to multi-disciplinary team meetings and liaise with colleagues across the sector to address complex issues related to health, social care, and well-being.
* Maintain accurate and up-to-date records in line with the Data Protection Act and confidentiality requirements.
* Prepare and submit detailed reports and case notes as required, adhering to local authority protocols and relevant legislation.
Qualifications and Experience Required:
* Degree in Social Work (or equivalent) from a UK university.
* Registration with Social Work England.
* Post-qualifying experience in adult social care, hospital discharge, or community-based care planning.
* Strong knowledge of Care Act 2014, Mental Capacity Act 2005, and NHS Continuing Healthcare (CHC) frameworks.
* Ability to manage complex casework and risk assessments.
* Strong problem-solving and decision-making skills.
* Experience working in a multi-agency environment, ensuring a coordinated approach to care.
