The job holder's key responsibilities will be Patient Care: Provide remote services to registered patients, offering advice on medicines, repeat prescriptions, and general pharmaceutical queries. Prescribing Support: Assist our primary care sitewith prescribing, audits, and managing high-risk drugs to ensure safe, effective patient care. Patient Assessment: Assess patients knowledge and socio-economic factors, tailoring advice to meet individual needs. Prescription Management: Prescribe urgent medications using summary care records to ensure accurate re-prescribing. Clinical Risk Management: Identify and manage clinical risks and health queries, ensuring patient safety and optimal care. Child & Adult Protection: Act on any child or adult protection concerns arising from health and medication calls, following set protocols. Collaborative Work: Work alongside multidisciplinary teams (MDT) to support service delivery and engage in quality improvement initiatives. Clinical System Documentation: Record accurate, timely information and advice within clinical systems and electronic databases. Regulatory Adherence: Uphold General Pharmaceutical Council standards of ethics, conduct, and performance in all aspects of practice. Professional Development: Participate in continuous professional development, working with colleagues to improve services and patient care. Location: Minehead Medical Centre (MMC) is a vibrant central hub for the town, with a dedicated team as close-knit as the rest of Minehead's population.Minehead Medical Centre is the only GP service in the area. The other two GP surgeries are located much further out on the outskirts. The patient list size is 12,300 with a slightly higher number of aging population when compared to the rest of Somerset.In recent years, Minehead Medical Centre has undergone refurbishment works to provide additional clinical rooms on the ground floor.Minehead is a coastal town in Somerset, lying on the south bank of the Bristol Channel and within easy reach of Bristol, Taunton, and Yeovil.
