Job summary We are looking to recruit to our Care Navigator Team If you have a keen attention to detail, office, customer service or GP Practice experience and a good sense of humour then this could be the role for you Hours Monday: 8.15am to 1.30pm Wednesday: 8.15am to 5.30pm with 30 minute unpaid break Thursday: 1.00pm to 5.30pm with 30 minute unpaid break Main duties of the job To answer the Practice telephone line and to ensure that patient requests for appointments, prescriptions, results and general enquiries are handled with care and discretion in a professional and business-like manner. About us The Cornerstone Practice is based in the centre of March in the heart of the Fens we have over 9600 patients. On staff we have 3 GP Partners, 4 Nurse Practitioners, a full time Mental Health Co-ordinator, Treatment Room Nurses, HCA's, a diabetic specialist Nurse, a COPD specialist Nurse and a large admin team. In addition we have social prescribers, Paramedics and a Pharmacist via our PCN that undertake clinics for our patients. Ensure that patients without appointments but who need urgent consultation are seen in a logical and non-disruptive way. Answer the emergency telephone. Explain practice arrangements and formal requirements to new patients and those seeking temporary cover, ensure procedures are completed. Inform patients of relevant charges, accept payment and issue receipts for same. Respond to all queries and requests for assistance from patients and visitors. Action requests for ambulance/car transportation and enter details appropriately on the system. Enter visits on to the visit list including all relevant information and where necessary refer to triage. Print of visits for the day. Become familiar with all appointment systems in effect including regular and incidental variations. Handling of specimens, blood tests, forms etc. taking into account Health and Safety measures.
