We are seeking an experienced Independent Social Worker to join our team in conducting Connect Person's Assessments at Grade 8 level. This role is crucial in ensuring that individuals receive the appropriate support and services tailored to their unique needs. The Independent Social Worker will be responsible for utilizing their expertise in social work principles, ethical standards, and best practices to conduct thorough assessments of candidates referred for support. Responsibilities
Conduct thorough assessments for Connect Person referrals in alignment with best practices and organizational guidelines.
Develop and implement individualized support plans based on assessment findings and client needs.
Collaborate with multidisciplinary teams and external agencies to ensure comprehensive service delivery.
Provide ongoing support and consultation throughout the assessment process, addressing any emerging issues for clients.
Document assessments, findings, and interventions accurately and within established timelines.
Advocate for the rights and best interests of clients, ensuring they are heard and supported throughout the process.
Stay informed about current social work practices, legislations, and policies to ensure compliance and best outcomes for clients.
Requirements
A relevant social work qualification and registration with the appropriate regulatory body.
A minimum of 3 years of experience in social work, particularly in assessments and client support.
Strong understanding of social work theories, principles, and ethical standards.
Excellent verbal and written communication skills for effective interaction with clients and stakeholders.
Demonstrated ability to work independently and manage a caseload effectively.
Strong assessment and analytical skills with attention to detail.
Ability to work collaboratively with various agencies and professionals to provide holistic support.
Hours Per Week: 36.00

End Time: 17:00
Pay Per Hour: £19.18
