A Vacancy at Greater Manchester Mental Health NHS Foundation Trust. The Home Based Treatment Team in Bolton provides an urgent care mental health service that offers a viable alternative to in-patient care in the community. We are seeking a highly motivated and ambitious band 5 social worker to progress their career by gaining invaluable experience and skills in an acute community setting.
The role will include providing crisis mental health care to those in the community, focusing on managing psychosocial needs that contribute to deterioration in mental health. The successful applicant will work with service users with a short-term focus on recovery from mental health issues, utilizing community resources. The service emphasizes a recovery/holistic model of assessment and treatment, which includes assessment and care planning for service users' acute mental health needs while supporting families and carers.
As a social worker within the team, you will predominantly focus on the social needs of service users, working closely with the MDT to facilitate appropriate signposting and clear escalation and management of safeguarding concerns. There is a strong focus on care planning and providing psychological interventions in the form of MAPS (maximising access to psychosocial interventions). The team implements safe and effective discharge planning, which includes expediting referrals to other services or to the care of GPs, discussed daily via the Multi-Disciplinary Team meeting with a focus on strong and supportive leadership.
