Stuart Road Surgery is continuing its journey through a period of transformation and we are looking for an Advanced Nurse/Clinical Practitioner to join our expanding clinical team to deliver excellent patient care. Newly qualified ANP/ACP's are welcome to apply and we are considering applications from those who are looking for around 16 hours (2 days) per week.

We are looking for an ANP/ACP. This will include home visits and may have the requirement to cover the duty rota as required.

Job responsibilities
DUTIES AND KEY RESPONSIBILITIES
* To provide care to patients at the point of entry to the health care system, including those with undifferentiated and undiagnosed health problems
* To provide continuing care to patients across the whole practice population
* To provide health maintenance and health promotional services to the well, offering educational supportive setting for clients to explore their health problems
* To provide psychosocial as well as physical care to the practice population
* To deliver an e-consultation and telephone consultation service, in addition to standard appointment types.
* To work as an autonomous clinical practitioner accountable for own professional judgement and actions
* To consult, assess, diagnose, plan, implement and evaluate treatment/interventions and agree goals for patients presenting with an undifferentiated diagnosis through a coproduction approach
* To consult, assess, diagnose, plan, implement and evaluate treatment/interventions and agree goals for patients with identified complex health and social care needs through a co-production approach
* To proactively identify patients at risk of developing a long-term condition (as appropriate) and facilitate the creation of their own goal setting through the development of a self-management plan
* To diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a patient management plan
* To undertake face to face assessments and develop treatment plans for patients not requiring a GP consultation
* Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within the scope of practice
* Request and interpret tests appropriate to patients needs
* Where required, undertake home visits as assessed by GP and acute visits for certain medical emergencies and arrange hospital admission for appropriate cases
* Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
* Undertake telephone triage work and telephone advice calls
* Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care
* Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation
* Assess, identify and refer patients presenting with mental health needs in accordance with the national and local guidelines for Mental Health services
* Implement and participate in vaccination and immunisation programmes for both adults and children
* Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment
* Communicate with and support patients who are receiving difficult or complex information or results in a professionally appropriate and supportive manner
* Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
* Ensure appropriate supervision of safe storage, rotation and disposal of vaccines and drugs
* Oversee the monitoring, stock control and documentation of controlled drug usage according to legal requirements where appropriate

Experience
* Experienced or newly qualified will both be considered
* Minor surgery / Joint injections would be desirable, but not essential.
Qualifications
* Valid NMC registration with supplementary prescribing.
* Minor surgery (or willingness to train)
