They are looking for a new member for their Referral & Assessment Team. Who we’re looking for: Our Referral & Assessment Team are looking for a highly skilled Locum Social Worker. By working in close partnership with families and other professionals to understand more about the support families may need to safely and effectively parent, our team’s clear aim is to identify where the appropriate support can be obtained. Adopting relationship-based practice enables us to strive to ensure all our children and families have meaningful and professional encounters with our Social Workers. The ideal candidate for this role will be dynamic and flexible with great inter-personal skills, excellent recording skills, and a sound knowledge of children’s legislation and thresholds – as well as the ability to work in a fast-paced environment and be able to multi-task. What will your responsibilities be? Collaborate within a supportive team alongside the Multi-Disciplinary Team (MDT), planning, monitoring, and evaluating work. Adhere to Social Work England's standards, professional values, and code of conduct, prioritizing the autonomy and choice of adults, carers, and families. Conduct needs-led assessments, complex care planning, and reviews, and initiate multi-disciplinary assessments as required. Provide social work support to individuals and their families, involving them in decision-making, and offering guidance and referrals as necessary to enhance social resilience and prevent hospital admissions. Practice according to the Professional Capabilities Framework (PCF) and uphold professional standards, demonstrating a commitment to ongoing development.
