Job summary An exciting bank opportunity has become available within the Cardiac Rehab team. The post holder must have previous Cardiac Rehabilitation experience. The post holder will work as a specialist nurse within the cardiology team contributing to the development, delivery and evaluation of a multidisciplinary, menu-based cardiac rehabilitation service. In addition, the post holder will participate in the development and delivery of education and training for other health professionals caring for patients with cardiovascular disease Main duties of the job Assessment of individuals' physical, psychological and social needs requiring advanced clinical reasoning, investigative and analytical skills. Planning, implementation and evaluation of specialised individual plans of care. This necessitates high levels of autonomy and advocacy. To work within trust wide clinical guidelines, policies and procedures, including cardiac rehabilitation service Making appropriate and timely referrals to other health care professionals and specialist services. Establishing early involvement and continuing support of patients and their families/carers, providing planned and spontaneous specialised advice, teaching and instruction to optimise rehabilitation potential and promote understanding of condition. Providing and evaluating specialist input to nurse/therapist led clinics and out-patient sessions, including exercise, education, relaxation and anxiety management components. This will include risk stratification, exercise prescription and leading/facilitating patient groups in the above components. Use of recognised outcome measures, assessment tools and advanced clinical skills to evaluate patient progress, reassess and alter interventions as required. Requesting appropriate investigations and the analysis, interpretation and comparison of the findings.
