Your Responsibilities: Lead and Inspire: Provide clinical leadership and day-to-day management of a team of nurses, healthcare assistants, and associate nurses, ensuring high standards of care and effective teamwork. Patient-Centered Care: Take responsibility for managing a caseload and delivering evidence-based nursing care, focusing on health needs assessments, complex care plans, and coordinated discharge planning to prevent hospital admissions. Collaboration Across Services: Work closely with GPs, social care, and other health and voluntary services to provide seamless, integrated care tailored to each patient’s unique needs. Who We’re Looking For: Qualifications: Registered General Nurse with current UK NMC registration and a Specialist Practitioner qualification in District Nursing (or working towards it). Experience: Proven experience in community nursing with a focus on complex care, team coordination, and multidisciplinary collaboration. Skills and Attributes: Strong leadership skills with the ability to motivate, delegate, and handle complex care decisions. Effective communication, problem-solving abilities, and a commitment to continuous learning are essential. Person Specification: Education/Qualification Registered Nurse with current UK NMC registration Evidence of continuing professional education and training at degree level Qualification in team / service management or prepared to work towards. Mentor or practice-based learning training. Community or Independent Nurse Prescribing qualification or prepared to work towards Post - registration qualification in district nursing or equivalent (desirable) Experience Evidence of recent post-registration nursing experience in community nursing Experience of negotiating and liaising within a multi-disciplinary team Experience of working with people with complex needs Experience of co-ordinating and managing a team Experience of teaching others and mentoring students Experience of evidence-based care Experience of undertaking comprehensive health needs assessment and developing complex care plans Experience of co-ordinating and delivering care for people with complex, long-term conditions.
