The Role
Based at Nicholson House, Maidenhead, this role offers flexibilityand autonomy. You'll spend much of your time in the community, visiting clients in their homes and making a tangible difference in their lives. You'll be responsible for managing your own caseload, working collaboratively with other disciplines in both statutory and voluntary sectors. While occasional office attendance is required for team meetings, training, and service-related needs, we embrace hybrid working practices.

Main duties of the job
* Participating as a member of a multi-disciplinary team managing a caseload of patients
* Undertake assessments of patients, involving their carers and family members in the patient's care and care plans.
* To be responsible for the formulation, execution, and evaluation of care plans for a designated number of patients who may present with significant complexity, in consultation with the patient and the multi-disciplinary team.
* Supervising Support Workers and/or students.
* Providing advice to other members of the service and agencies on the needs of service users.
