Job summary An exciting opportunity has arisen for a part time Band 5 Bone Health Nurse to join Nottingham CityCares Bone Health Service. The team operate Monday to Friday, 08.00- 16.00, this post will be for 15 hours, and we can consider flexible working for example school hours. The team are based at Bulwell Riverside. The Bone Health Service aims to be a proactive, forward thinking, and dynamic service which provides evidence-based assessment and follow up for individuals requiring ongoing Osteoporosis management in the community and those who are at risk of frailty fractures. To accept clinical responsibility for a designated caseload of patients and to organise this effectively regarding clinical priorities and use of time. To act as a link between primary (GPs) and secondary care (Specialist Physicians) to deliver seamless care for patients within the specialist area. To participate in the Bone health Virtual Clinics with the community specialist Consultant. To attend case conferences/CDG meetings within the link neighbourhood areas that have been allocated.
