Minimum Requirements The post-holder will take responsibility for their own development, learning and performance. This is a role taking advantage of the NHS Network Contract DES for Additional Roles Reimbursement Scheme. Enrolment (or completion) of the CPPE Primary Care Pathway is expected. Working towards being a pharmacist independentprescriber within 3 years. Meet the GPhC CPD requirements for registration on an annual basis. Complete any internal training required by the PCN. Main Duties Achieves efficiencies and effectiveness within the prescribing activities of the PCN group of GP practices. Promotes high quality cost-effective and evidence-based use of medicines within the PCN. Be a contributing team member to the PCN Pharmacy Team, including 3 Clinical Pharmacists and 2 Pharmacy Technicians to respond to medication-related queries generated by patients, GPs, other health care professionals and staff. Assist the PCN Pharmacy Team to develop and ensure a safe efficient medicines optimisation programme including repeat prescribing, medication review and medicines reconciliation services. Undertake Structured Medicine Reviews (SMRs) in line with Network Contract DES either face to face or via telephone support on identified stratified-risk patients that may include residents in care homes, in their own, with long-term conditions, and/or learning disabilities. Be proficient with TPP/SystmOne GP prescribing system and supporting IT to record personally generated information and maintain a database of information relating to the work done in the practices. Identify and produce recommendations for medication optimisation for clinical pharmacists, GPs and other multi-disciplinary team members. Work with care home staff to improve safety of medicines ordering and administration if required. Encourage good management of common/minor/self-limiting ailments, including promotion of self-care and homely remedies. Contribute to the safety of drug monitoring processes and drug alerts. Analyse, interpret and present medicines data to highlight issues and risks to support decision making. Advocate and implement local, regional and national formulary recommendations, including public health campaigns. Liaise with all members of the local PCN including community pharmacies and community nursing team, secondary care providers and regional CCG teams on prescribing-related matters. Aim for good collaborative working practices and develop relationships with other professionals across PCNs and the wider health and social care system. Undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities. Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety. Respond to the changing NHS landscape, including contributing to Covid-19 vaccination programme, for example becoming a vaccinator and/or vaccine manager. Duties will vary according to the demands on the PCN as agreed with the PCN Clinical Director and Lead PCN Clinical Pharmacist.
