Community Mental Health Practitioner - Memory Service

An exciting opportunity has arisen for a Senior Community Mental Health Practitioner to join our Memory Team in Scarborough, based at Cross Lane Hospital. We are looking for a dynamic, positive and forward-thinking clinician with a passion for caring for people with cognitive impairment, their families, and carers, and with a specialist interest in delivering high quality, effective and place-based Memory Services. This is an exciting time to join our team and locality - this role is an important part of a dynamic improvement review of how we deliver our memory services to broaden access and ensure delivery of specialist memory assessment in a timely manner. This is essential to enable timely diagnosis of dementia and associated needs and conditions, which supports access to early treatment and support, decision making, and aids optimal management and life outcomes for those in need.
Candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework.

Main duties of the job
The ideal candidate will be highly motivated and be able to demonstrate best practice in community mental health service delivery for older people, their families, and carers. The ability to work independently, effectively and collaboratively within Multidisciplinary Teams, with external partners, and most importantly - with people, their families and unpaid or paid carers, is an essential core competency.
As Senior Practitioner, you will support the team's triage and duty system as part of a rolling rota. You will also be responsible for making the initial contact to support service users and their families and carers in their understanding of the memory pathway. You will undertake a detailed history using a holistic memory assessment framework, with the initial collation of all clinically relevant individual service user information (clinical history) and involving families and carers as part of our genuine commitment to co-creation. In this way, you will promote positive experience and clinical outcomes from the beginning of the person's service journey.
There is a requirement to accurately record and document clinical discussions and assessment details on our clinical system and to ensure that all individual service user documentation is up to date and available to inform internal clinical meetings and aid consistent, safe and high quality decision making by members of the MDT and wider colleagues.

The team is based in the same office space and works closely with the Scarborough Community Mental Health Team for older people, so you will benefit from extensive MDT support and input in your role.

In return, you will receive regular appraisal, clinical, management and caseload supervision and will benefit from the support of an experienced and committed multi-disciplinary leadership team, including the Team Manager and the Clinical Specialist Nurse/Advanced Nurse Practitioner.

Person Specification
Qualifications
* Current professional registration with an approved professional body.
* Evidence of Continuing Professional Development.
* For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education.
* 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework.
* Recognised sign-off mentor, clinical educator or equivalent. Must be achieved within agreed timescale.
* Key skills in literacy, numeracy and ITQ level 2 (or equivalent).
* Post-graduate qualification in a relevant area.
* For registered nurses: Non medical prescriber.
Experience
* Significant experience working with people with mental ill health in a community setting.
* Working collaboratively with service users and their families/carers.
* Providing clinical supervision.
* Leadership or management experience.
Knowledge
* Demonstrable knowledge to post-graduate level of evidence-based practice in caring for patients in the designated field.
* Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act).
* Clinical Risk Assessment and Management and its application in practice.
* Detailed understanding of Safeguarding and its application in practice.
Skills
* Communicate complex and sensitive information effectively to patients, carers/families and all members of the multidisciplinary team.
* Work effectively as part of a multidisciplinary team and undertake lead professional and care co-ordinator responsibilities.
* Provide leadership and monitor, co-ordinate and prioritise the activities of a team.
