Job summary HUC provides the NHS111 and Out of Hours GP service to over 4.5 million patients across Hertfordshire, Cambridgeshire, Bedfordshire, West Essex and the South West of England. The Advanced Clinical Practitioner plays a pivotal role in delivering high-quality, patient-centred care in a dynamic healthcare environment. As an experienced clinician, you will assess, diagnose, and manage a wide range of acute and chronic conditions while integrating evidence-based treatments and holistic patient care. Working autonomously and collaboratively within a multidisciplinary team, you will contribute to improving patient outcomes by providing expert clinical decision-making, advanced diagnostic skills, and proactive management of long-term health conditions. This role requires a strong commitment to patient advocacy, health promotion, and preventive care. You will engage in complex clinical assessments, prescribe treatments, and offer expert guidance on medication management, vaccinations, and lifestyle interventions.
