Job overview

The holder of this post will work principally with the Orthogeriatric Consultants, but will be expected to become an integral member of the whole trauma & orthopaedic team.
Main duties of the job
1. Providing excellent, holistic and multi-disciplinary inpatient care.
2. Comprehensive Geriatric Assessment of patients as part of a multidisciplinary team.
3. Providing the necessary communication back to GPs and family members.
4. Being involved in decision-making regarding care arrangements for older patients which may include case conferencing, best interest meetings and family meetings.
5. The management of Orthopaedic patients (over the age of 60) with medical problems.
6. Co-ordinating care with other members of the wider team including anaesthetists, dietician, physiotherapy, occupational therapy and SALT.
7. Links with STARRS and Harrow Rapid Response focusing on prevention of Hospital re-admission, ambulatory care, acute medicine, A&E and outpatient services for older people.
8. Working to reduce the number of re-admissions to hospital for Older People and the length of stay of Older People at both Northwick Park and Ealing Hospitals.
9. Attend the weekly orthopaedic teaching, medical grand rounds and FY1/FY2 teaching. You may be required to teach medical students.
10. Making presentations at meetings and undertake audit and other clinical governance activities.
11. Being actively involved in teaching and training within the department.

Detailed job description and main responsibilities
Early identification of new patients admitted each day, who fall into the following categories:
1. Pre-op >60 years with fractures (ex NOF).
2. Those >60 years under orthopaedics who are unwell with medical problems.
Arranging/completing the following as part of the Complex Geriatric Assessment:
1. Appropriate screening bloods, ECG, Falls review, Bone health review, Medication review and rationalisation, 4AT on admission and during IP stay, Consideration of further investigations (CXR, CT etc), Social history and collateral.
