Job summary The post holder will have responsibility for developing and supporting the speciality service to provide equitable care to patients presenting with lymphoma Cancer and act as a resource to carers, the multidisciplinary colleagues and the PHCT. This will be achieved by the post holder ensuring a smooth transition of care from initial referral through diagnosis treatment and beyond treatment into the the survivorship stage, establishing links between the acute and primary care group, supporting both patient and carers to ensure a robust stratified follow up process for cancer patients. The post holder will liaise with services within Wales and with other key partner such as Health Boards, Primary care, Voluntary services, Social Services and Hospices. The post holder with provide expert nursing service and support to patient and their families at all stages of disease. The main responsibilities are related to clinical expertise with an emphasis on initiating, performing and maintaining standards of clinical excellence. Main duties of the job Utilise advanced nursing skills to undertake specialist assessments, care plans. critically analyse complex information Nurse prescribing Clinical Practice Develop and Implement policies and clinical pathways to improve care Practise autonomously in line with best practice ensuring highly specialist advice Ensure consultations and changes in treatment are clearly communicated Assess and critically analyse complex health information to implement and evaluate ongoing care management plans ensuring prompt, effective appropriate patient care. To be the lead clinical link between specialist services and other care providers Communicate effectively and interpret complex information to support informed patient decision making and consent providing empathy and reassurance while ensuring any barriers to understanding have been overcome Health promotion To provide psychological and practical advice/support to patients' carers/ families whilst respecting patients' confidentiality The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
