Intensive Floating Support Worker Hours: 35 per week (Full time) Contract: Permanent (subject to on-going funding) Work-based contract Salary: £24,000 Shift Pattern: Generally, Monday-Friday, with occasional evening and weekend work. Flexibility on start and finish times to support a work-life balance. Location: Newport Compulsory participation in the 2nd Tier out of hours on-call rota All team members must work flexibly on occasions to meet the needs of the Charity and the people we support. Purpose of the post: To offer intensive floating support to people with multiple co-occurring needs including challenges linked to substance use, homelessness, mental health, giving particular focus to women who may be experiencing or be at risk of, abuse, harm or violence. You will work closely with the Newport Gateway Team (who will coordinate this service) and the other partner agencies engaged on this initiative striving to meet the housing related support needs of the people engaged, so that homelessness can be alleviated, and accommodation sustained for the long term. You will work closely with your colleagues at Cyfannol ensuring people referred to this service have ready access to all our services, including the Hwb drop in facilities, Horizon sexual violence services and our domestic abuse services. Specific Responsibilities Collaborate with Newport Gateway Team who will coordinate and manage this new initiative, providing all referrals and monitoring progress Collaborate with the other providers involved in this initiative, working together when requested to support the achievement of the best outcomes for people recieving support Provide a trauma informed, person led and responsive floating support to people referred to this service Provide people with support needs assessment, risk assessment, safety planning and support planning services Provide support around identifying and managing any safety and safeguarding concerns and support people to contact any required statutory bodies such as the police or social services when required Attend case conference meetings and Multi-Disciplinary Team meetings in relation to the people supported Provide support to people to find, access and retain appropriate accommodation and in doing so, support people with all aspects of 'housing related support' in line with the service specification, which includes : Budgeting, organising finances, debt management and accessing welfare benefits Managing household correspondence Developing living skills which promote independance Developing support networks and positive social relationships Liaise with internal colleagues, ensuring people have ready access to all of Cyfannol's hollistic and integrated domestic abuse and sexual violence services Provide signposting and referral to external statutory and non-statutory agencies, when people supported wish to be referred into their services Work closely with the Housing Advice and Prevention team and the service user, supporting them to act and recieve support in line with individual's personal housing plan Access education, training, volunteering and employment opportunities Address issues that impact on their physical health or mental health Developing skills to identify potential risks and maintain or improve the safety of themselves, their property and others Update case management system(s) Contribute to research, consultation, surveys as required to improve services for people with complex needs General Responsibilites: Contribute to the: Development and achievement of our Vision, Mission and Objectives Promotion of our values, culture and aspirations Creation of a safe, supportive and explorative working environment Promotion of Equity, Diversity and Inclusion Upholding of our Policies and Procedures It may be necessary, following consultation, to amend the job description in light of experience and changing circumstances A current valid driving licence, access to a car and insurance covering business use is essential.
