Homeless Health Outreach Advanced Nurse Practitioner
Heart of Bath is looking for a part-time Homeless Health Outreach Advanced Nurse Practitioner. We are a practice based across 2 sites in the City of Bath, serving a diverse team of clinical and non-clinical staff who work together to provide care for our 26,500 patients and support Julian House.
The post holder will be responsible for providing health assessments and nursing care for homeless individuals with long-term health issues who are not regularly accessing primary care services.
The role includes ensuring patients are registered with primary care services and are supported to attend appointments to meet their health needs and access preventative care.
Main Duties of the Job
* Provision of appropriate and regular screening assessments based on current research related to the health needs and problems of homeless individuals.
* Management of chronic diseases.
* Health care triage, assessment, and treatment.
* Patient advocacy, referral/linkage to primary care, and support for medication adherence.
* Health education, support for chronic health conditions, and self-management goal setting.
* Deliver proactive promotion of health services to the local homelessness community, ensuring awareness of available services.

Person Specification
Qualifications
* Qualified Advanced Nurse Practitioner.
Experience
* ANP experience.
* Outreach experience is advantageous.
