Hours per week: 08.00am - 17.30pm
Location: Community based (you might be asked to complete night shift sign-offs, spot checks), WFH, 1 day a week in Liverpool St office preferably on Tuesdays
Aim of the role
As an essential member of our Homecare services team, your work will be closely aligned with our managers to ensure the highest standard of person-centred care. The Clinical Lead Nurse is responsible for ensuring that all care services are delivered in line with regulatory requirements, clinical standards, and best practices. Key responsibilities will encompass:
1. Clinical Leadership
o Provide clinical leadership and guidance to nursing staff, ensuring safe, high-quality care.
o Serve as the point of contact for the multi-disciplinary team, patients, and their families regarding clinical issues.
o Develop and implement clinical policies, procedures, and best practices in collaboration with the management team.
o Monitor and evaluate clinical performance, patient outcomes, and adherence to care standards.
2. Patient Care
o Conduct initial assessments of new service users.
o Ongoing care plan reviews and risk assessments, revise care plans as necessary, collaborate with other healthcare professionals.
o Observe care staff and ensure their competency.
o Respond to emergencies and complex patient needs, offering expert advice and interventions.
o Occasionally provide direct care to service users to maintain hands-on clinical skills (e.g., tracheostomy change/sign off carer competency at the same time).
3. Compliance & Quality Assurance
o Ensure compliance with all legal, regulatory, and ethical standards, including Care Quality Commission (CQC) requirements.
o Lead clinical audits, risk assessments, and quality improvement initiatives to enhance care quality and safety.
o Investigate incidents, complaints, or clinical concerns, implementing corrective action where necessary.
o Keep up to date with developments in nursing practice, health care, and legislation to ensure ongoing compliance with best practices.
4. Training & Development
o Identify training and development needs of nursing and care staff and implement training programs to address those needs.
o Ensure that all staff are trained and competent in the use of equipment, techniques, and care interventions relevant to their role.
5. Operational Support
o Work closely with the Homecare Manager to ensure the effective and efficient delivery of care services, managing resources, and ensuring staffing levels are appropriate.
o Contribute to the operational aspects of the division, including business development and liaison with commissioners, attending MDT meetings.
o Act as a liaison between external healthcare providers, including hospitals, GPs, and specialists, to ensure continuity of care.
6. Communication & Relationship Management
o Foster a positive, supportive working environment, promoting teamwork and collaboration within the care team.
Essential Qualification & Experience
* Registered Nurse with a valid NMC registration.
* Significant experience in homecare, community nursing, or clinical settings, particularly with complex care needs.
* Excellent clinical assessment skills, with the ability to develop comprehensive care plans.
* Strong understanding of safeguarding, risk management, and infection control practices.
* Up-to-date with the latest healthcare regulations, clinical practices, and care guidelines.
