Main duties of the job
The Social Worker role at Elysium Healthcare's St Mary's Hospital in Warrington involves providing specialized services for people with Acquired Brain Injuries (ABI), progressive neurological conditions, Autism Spectrum Conditions (ASC), and Deaf Mental Health problems. Job responsibilities
As a Social Worker, you will manage a complex caseload and be responsible for social care assessments, risk assessments, facilitating group activities, and contributing to ICR meetings, supporting service users to make viable plans for their future.
You will report to the Safeguarding Lead and manage internal investigations, liaising with home authorities for service users during their admission and discharging process, while enjoying access to a range of training opportunities that can help your career grow and achieve your aspirations.
As a Social Worker you will be:
* Making referrals to LADO and Local Authority in line with safeguarding and assessments.
* Educating Hospital staff about relevant safeguarding matters.
* Completing a full annual social care reassessment within the CPA format.
* Facilitating access to welfare rights, housing, advocacy services and associated support services.
* Participating in 1:1 supervision.
To be successful in this role, you'll need:
* A Social Work England registration.
* A Social Work degree.
* Experience within a similar environment.
