Job summary Horton Park Medical Practice is looking to appoint a motivated, enthusiastic, experienced Practitioner to join our friendly clinical team. The Advanced Nurse Practitioner/Advanced Care Practitoner will work autonomously within their profession scope of practice providing acute care for patients via face to face, video and telephone consultations and when required, home visits. They will need to provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care, making necessary referrals for investigations in the appropriate manner. Main duties of the job The candidate will need to demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.
