Job summary
This post is a developmental role, starting as an 8a the successful candidates will be supported to further develop skills necessary to the role of a Nurse Consultant. There will also be an expectation the successful candidates will undertake further study up to Masters level in an area relevant AMH Community. Progression to 8b will be dependant on completion of set criteria.
Community mental health services play a crucial role in delivering mental health care for adults and older adults with severe mental health needs as close to home as possible.
The NHS Long Term Plan and NHS Mental Health Implementation Plan 2019/20 - 2023/24 set out that the NHS will develop new and integrated models of primary and community mental health care. Community based offers will include access to psychological therapies, improved physical health care, employment support, personalised and trauma informed care, medicines management and support for self-harm and coexisting substance use.
By 2023/24, this will enable at least 370,000 adults and older adults per year nationally to have greater choice and control over their care, and to live well in their communities. Our community services are expanding and transforming, and we are keen for additional clinical leadership, governances and oversight to support our workforce into these services.
There are two substantive posts and one 2 year fixed term post available.
Main duties of the job
The post holder will work autonomously to manage their own workload and continue to develop and apply their knowledge and skills to a broad range of clinically and professionally challenging and complex situations.
Acting within their professional boundaries the post holder will be responsible for patients across defined community pathway and this is not limited to, but will involve:
1. Initial history taking
2. Clinical assessment
3. Diagnosis
4. Determining appropriate treatment options
5. Delivery of care which is personalised
6. Evaluating response to treatment.
They will demonstrate safe, clinical decision-making and both directly provide and supervise other members of the multi-disciplinary team in the delivery of expert care to patients within the defined clinical area.
The post holder will have direct patient contact for 60% or higher of their job plan providing appropriate intervention to a patient caseload within the specialty from diagnosis throughout the treatment trajectory.
The post holder will support the Consultant Psychiatrist and existing Nurse Consultants in providing clinical leadership to the team. This will involve establishing partnerships internally and externally with a common purpose across agencies to impact on service development. This will be in accordance with national drivers and local needs assessment to create an impact locally and nationally.
