Other responsibilities include:
1. Using telephone triage to assess patient and family needs, providing advice and support.
2. Offering face-to-face assessments to provide the best and appropriate care for patients at the end of life, meeting their choices and supporting their families/carers throughout the patient's journey.
3. Taking a clinical lead within St. Luke's Hospice Community Services, working at a specialist level, and ensuring standards are maintained throughout the team.
4. Providing high standards of nursing care for palliative patients according to their assessed plan of care/need.
5. Conducting initial assessments and ongoing reviews to meet the needs of patients in various settings: home, hospital, care home, and hospice.
6. Being fully aware of responsibilities and processes in safeguarding vulnerable adults and children.
7. Promoting personal and professional development of all nursing staff.
