Job summary An exciting opportunity has arisen for a part-time (22.5 hours) Band 7 Heart Failure Clinical Nurse Specialist to join the Pembrokeshire Community Team. We are looking for an experienced registered nurse with a high level of demonstrable clinical skill and theoretical knowledge in heart failure. The team deliver expert nursing care to people diagnosed with heart failure in community settings. There is a focus on multidisciplinary care planning and initiating and monitoring specialist interventions to reduce hospital admissions. The team work closely with cardiology consultants, multi-professional cardiology colleagues, primary care and community- based nursing teams/services with the ultimate aim of improving patient outcomes and wellbeing. The successful candidate will be able to demonstrate high-level Heart Failure nursing knowledge and skills, they will possess excellent verbal/written communication skills, be organised and have experience of managing a community caseload/clinic. A post-registration Heart Failure qualification is essential, and completion of (or willingness to undertake) non-medical prescribing (V300). Main duties of the job To expertly and proactively manage a defined caseload of people with highly complex needs using evidence based/client centred principles, and to assess, plan, implement and evaluate specialist interventions across the primary/secondary care interface. Work with the wider cardiology team and clinical/service delivery managers to develop and drive Heart Failure Services across the Health Board. The post-holder will be a driver for change. Assess, plan, implement and monitor care, deliver specialist nursing care, and maintain accurate health records for the community caseload. The post-holder will work collaboratively with members of the Heart Failure multidisciplinary team to ensure that holistic health needs are met. This includes working with Heart Failure In-Reach Clinical Nurse Specialists, Community and primary Care, Consultants and Cardiologists. Work autonomously in patients' homes, community dwellings, community clinical premises and hospital. Using high-level communication skills, the post-holder will foster positive therapeutic relationships. They will deliver education; providing expert support, advice and acting as an educational resource to colleagues. The post-holder will demonstrate the Health Board's values.
