Clinical Responsibilities: Work autonomously and be accountable for his/her own professional actions within the limits dictated by their own Professional Training Body and to know the limits of their abilities and consult with others such as the practice GPs when appropriate. Undertake duties at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary care services. Assess undifferentiated patients with a range of acute, non-acute and chronic medical conditions. Take history, make appropriate physical examinations, formulate differential diagnoses and implement management treatment plans with relevant follow up. Where appropriate, make referrals to other health professionals within the practice, in the wider primary care team and to secondary care, arranging hospital admission if appropriate. Undertake telephone triage work and telephone advice calls with appropriate safety netting. Be involved with practice developments in e-consultations. Prescribe medication as necessary under the non-medical prescribing scheme and use the formularies agreed by the practice and Medicines Optimisation (ICS). Record all prescribed medication in the clinical record and diligently check for contra-indications and interactions. Ensure that dispensing patients have the opportunity to receive their medications from the practice dispensary. Supply and administer medications as appropriate via PGD. Seek advice from practice pharmacist or GP regarding medication queries. Undertake regular medication reviews and where appropriate align any changes to medications indicated in hospital letters with the clinical medical record. Request and interpret tests appropriate to patients needs. Make sure that necessary actions arising from these investigations are completed using Tasks on SystmOne. Maintain accurate record keeping within SystmOne and carefully Code all clinical encounters, keeping the medical summary and Major Active problems up to date. To safely and effectively prescribe medication to patients, as an independent prescriber.
