The Benefits and Homeless Prevention Service (BHPS) Complex Needs Team provides holistic assistance to Hackney Residents who present as homeless either from various institutions or those who approach the Council directly for homelessness assistance. This role sits within this multidisciplinary team alongside with splitting their time between Council buildings and Homerton wards.
This is an exciting and unique role, in which you will play an integral role ensuring patients being discharged from the mental health wards have all the relevant assessments carried out, and that the necessary care plans and risk assessments are available for the BHPS to be able to make informed and safe decisions on any temporary or settled housing options.
This role will involve building strong working relationships between ELFT mental health services and temporary accommodation while also working closely with service users, their families and other relevant support networks. You will provide a high quality and effective mental health social work service to residents that are in housing need. You will respond to the needs of adults with mental disorders and their carers in a timely manner, within the boundaries of statutory legislation, and with regard to individual circumstances, combined with assessing needs, using a strengths-based and person-centred approach. Planning and delivering outcome-focused interventions linked to promoting independence & social inclusion will also be part of your daily remit.
Minimum Requirements:
1. Significant experience of working in an adult mental health setting or delivering a mental health service.
2. Knowledge of housing and homeless legislations.
3. Qualified AMPH’s or social workers who are willing to undertake training in this area.
4. Qualified in CQSW or DipSW or equivalent, HCPC registered.
5. Experience of working in inner city urban areas.
