The Fremantle Trust has an exciting opportunity for a Relief Waking Night Team Leader to join our team on a relief basis, based at our service in Farnham Common.
Principal Tasks Include:
1. Provide respectful high-quality support and care for people who use Fremantle services in accordance with their individual Support Plans.
2. Line manage the night staff team ensuring effective performance management and deployment of staff for a defined group within the service, to ensure that they provide excellent care for people.
3. Ensure Night Care Plans are in operation and regularly reviewed.
4. Practically assist people with tasks of daily living such as personal care and mobility, and end of life care, as directed in the Support Plan.
5. Support people to take their medication as prescribed and recorded in the individual Medication Administration Record.
6. Ensure that people have companionship and social, emotional and leisure support in accordance with their wishes and the Support Plan.
7. Ensure that people receive planned or urgent health care whenever required.
8. Participate in and ensure that domestic tasks e.g. laundry, cleaning and tidying are undertaken.
9. Ensure that the building is secure and safe during night hours and deal with any emergency situation that may arise.
10. Record essential information in service users’ daily reports, care and health monitoring charts, accident and incident reports, and service message systems clearly, accurately and immediately.
11. Implement Fremantle’s Performance Development and Support Policy and ensure that all staff receive comprehensive induction, probationary period, performance and development reviews, key sessions and participate in the in-house staff development programme.
12. Ensure that day-to-day tasks and routines are carried out to contribute to the smooth running of the service as a whole.
13. Actively promote the service and Fremantle by clear, respectful and helpful communication with all potential customers including people who use services, relatives, health and social care professionals, other officials and contractors, visitors as well as Fremantle colleagues.
14. Contribute to the success of the whole staff team by respecting colleagues and participating in team meetings and communications systems.
15. Any other duties consistent with the business of Fremantle, the job, and hours of work as may reasonably be required.
