Job summary
:
The practice nurse will work as part of our primaryhealthcare team providing General Medical Services to the patients of the practice.
Job responsibilities:
1. Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs
2. Cytology
3. Wound care / ulcer care/Doppler
4. Removal of Sutures
5. Health Checks
6. New Patient Medicals/Urinalysis
7. ECGs
8. Venepuncture
9. Hypertension Management
10. Routine immunizations
11. Requesting pathology tests, for example urine culture, swabs
12. Following agreed clinical protocols with referral to senior nurses or GPs as appropriate
13. Clearing and re-stocking consulting rooms
14. Vaccine/cold chain storage monitoring and recording.
15. Other tasks will include
16. Maintaining and cleaning equipment used by the nurses and GPs
17. Maintaining GP and Nurses rooms, stocking and rotating items as required
18. Chaperoning and assisting patients who are being examined by another clinician.
19. Maintain general tidiness and cleanliness of nurses and treatment rooms
20. Ordering of vaccinations to maintain stock levels
21. Participation in administrative systems in the Practice
22. At all times there will be a need to maintain accurate records
23. A duty to advise senior nurses of potential problems or errors within the range of assigned tasks
24. Attend and participate in any Practice meetings when required.
25. Any other delegated duties appropriate to the post
Patient health checks
26. Identifying significant abnormalities
27. Obtaining appropriate information using a lifestyle questioning approach and identifying appropriate health promotion issues
28. Working with patients to develop a management plan where health problems or potential health problems are identified

Health & safety:
The post-holder will implement and lead on afull range of promotion and management their own and others health and safetyand infection control as defined in the practice Health & Safety policy,the practice Health & Safety manual, and the practice Infection Controlpolicy and published procedures. This will include (but will not be limitedto):
32. Using personal security systems within the workplace according to practice guidelines
33. Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines
34. Responsible for the correct and safe management of the specimens process including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements
35. Management and maintenance of Personal Protective Equipment (PPE) for the practice including provision, ordering, availability and ongoing correct usage by staff
36. Responsible for hand hygiene across the practice
37. Ownership of infection control and clinically based patient care protocols, and implementation of those protocols across the practice
38. Active observation of current working practices across the practice in relation to infection control, cleanliness and related activities, ensuring that procedures are followed and weaknesses / training needs are identified, escalating issues as appropriate
39. Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks across clinical and patient process
40. Making effective use of training to update knowledge and skills, and initiate and manage the training of others across the full range of infection control and patient processes
41. Monitoring practice facilities and equipment in relation to infection control, ensuring that provision of hand cleansing facilities, wipes etc are sufficient to ensure a good clinical working environment. Lack of facilities to be escalated as appropriate.
42. Safe management of sharps procedures including training, use, storage and disposal
43. Using appropriate infection control procedures, maintaining work areas in a tidy, clean and sterile, and safe way, free from hazards. Initiation of remedial / corrective action where needed or escalation to responsible management
44. Actively identifying, reporting, and correction of health and safety hazards and infection hazards immediately when recognised
45. Keeping own work areas and general / patient areas generally clean, sterile, identifying issues and hazards / risks in relation to other work areas within the business, and assuming responsibility in the maintenance of general standards of cleanliness across the business in consultation (where appropriate) with other sector managers
46. Undertaking periodic infection control training (minimum twice annually)
47. Routine management of own team / team areas, and maintenance of work space standards
48. Waste management including collection, handling, segregation, container management, storage and collection
49. Spillage control procedures, management and training
50. Decontamination control procedures, management and training, and equipment maintenance
51. Maintenance of sterile environments

52. Must have experience working as part of a nursing team
53. Must have knowledge of QOF and chronic disease management.
Qualifications

54. Experienced practice nurse with knowledge of QOF and chronic disease management.
55. Qualifications in Asthma and COPD are desirable.
