Responsibilities
Conduct comprehensive assessments of clients' needs and circumstances.
Develop and implement individualized care plans for adult clients.
Provide emotional support and counseling to clients and their families.
Advocate for clients' access to necessary services and resources.
Collaborate with healthcare providers, community organizations, and other stakeholders to coordinate care.
Monitor clients' progress and adjust care plans as needed based on their feedback and outcomes.
Document all interactions, assessments, and interventions accurately in accordance with policies and regulations.
Requirements
Master's degree in Social Work from an accredited institution.
Current licensure as a social worker in the state.
Experience in working with adult populations and knowledge of relevant issues.
Strong interpersonal and communication skills, both written and verbal.
Ability to work independently and as part of an interdisciplinary team.
Proficient in using case management software and documentation practices.
Commitment to ongoing professional development and staying informed on best practices.
Hours Per Week: 35.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £22.60
