We have an exciting opportunity for an experienced Enhanced Nurse Practitioner to join our established Primary Care Network (PCN). We serve the people of South Somerset West PCN, with a population of approximately 37,000 patients. We are an expanding and progressive organisation with excellent patient care as our central value.

Buttercross (Somerton) and Ilchester Surgeries
Hamdon Medical Centre
The hours available are from 25 hours per week (part time) up to 37.5 hours per week (full time). Working pattern to be discussed at interview.
The salary is £58,465.34 -£61,820.74 pro rata, per annum, depending on experience with a NHS or NEST Pension offered.
Interviews will be week commencing 4th December 2024.
Main duties of the job
The post holder will be an experienced Nurse Practitioner working at enhanced level within primary care who is able to deliver quality individualised care to our patients. You will be expected to utilise your skills in clinical assessment and diagnostics reasoning to make decisions to determine and initiate changes to treatment. A keen interest in acute Frailty or Urgent Care would be advantageous. The Enhanced Nurse Practitioner is funded through the ARRS funding scheme and as such there are core requirements for the individual as listed below.
* The Enhanced Nurse will have a Level 7 Post Graduate Certificate in Clinical Practice along with:
* Full NMC Registration with no restrictions
* Non-Medical Prescriber V300
* Clinical Examination and Diagnostics Level 7
* Urgent Care Level 7 module
* Experience of working in urgent care and/or primary care

MAIN DUTIESAND RESPONSIBILITIES
The following are the core responsibilities of the Enhanced NursePractitioner. There may be on occasion a requirement to carry out other tasks;this will be dependent on factors such as workload and staffing levels. Youwill work within your own scope of clinical practice and:
Work alongside themulti-disciplinary team (including GP support) in accordance withclinically based evidence, NICE guidance, the Direct Enhanced Servicescontract of the Network.
To lead on the vaccinationprogramme for SSWPCN in alignment with the practices and the clinicalteams within the practices.Providing advice, support and guidance alongside leading thehousebound and care home vaccination provision.
Provide leadership and oversight tothe community investigation hub (CIH).
As required, be able to prescribefor the caseload of patients within your proven scope of practice. Usingyour knowledge, you will provide information and advice on prescribed orover the-counter medication on medication regimens, side-effects, andinteractions.
Be able to perform investigatoryprocedures, including laboratory and radiology.
Communicate proactively andeffectively with all colleagues across the multi-disciplinary team,attending and contributing to meetings as required.
There may be a requirement toundertake home visiting and triage duties when required for the PCN (thiswould be in liaison with the practices within the PCN)
Collaborate with other members ofthe PCN including doctors, nurses and AHPs, accepting referrals, usingclinical reasoning skills to recognise when referral to them forspecialist care may be required.
Maintain accurate andcontemporaneous health records appropriate to the consultation, ensuringaccurate completion of all necessary documentation associate with patienthealth care and registration with the practice.
Clinically examine and assesspatient needs from a physiological and psychological perspective and planclinical care accordingly.
Prioritise health problems andintervene appropriately to assist the patient in complex, urgent oremergency situations, including initiation of effective emergency care.
Support patients to adopt healthpromotion strategies that promote healthy lifestyles and apply principlesof self-care.
Enhance their own performancethrough continuous professional development, keep up to date with currentevidence-based practice
Impart own knowledge and skills toPCN colleagues to meet the needs of the service.
Work in partnership with otherclinical teams to collaborate on improving the quality of health care byresponding to local and national policies and initiatives as appropriate.
Participate in clinical audits,research projects, and implement changes as required, including thedevelopment, and updating of practice protocols / guidelines andprocedures locally.
Participate in the review ofsignificant and near-miss events.
Take personal responsibility forown learning and development, including the requirement to maintaincurrency, achieving all targets set in own Personal Development Plan(PDP).
