Community Staff Nurse - North West Locality
The focus of District Nursing is to support patients towards self-management, enabling them to continue to live as full and productive a life as possible, through education, equipment and or lifestyle changes. We also have the privilege of supporting people and their families at the end of their lives, working in close liaison with the Hospice Teams and Specialist Palliative Care Nurses.
Main duties of the job
Providing a high standard of care to patients in their own home or clinic environment, this will include a number of clinical competencies:
1. End of life care - syringe driver and symptom management.
2. CVAD management
3. Catheters - male, female and suprapubic
4. Complex wound care
5. Drain management
6. Compression/Dopplers
7. Bladder Scanning
8. Holistic assessment of needs
9. Skin Integrity
There will be training provided via the Integrated Care Academy for the successful candidate.

Job responsibilities
1. To demonstrate up to date knowledge of national and local professional issues and to lead/facilitate the implementation at a local level.
2. To undertake any other duties, required by the line manager appropriate to the grade and relevant to the post.
3. To act on behalf of patients, carers and staff to ensure their views are represented.
4. To implement the local and national Clinical Governance agenda.
Person Specification
Education & Qualifications
* Registered Nurse Level 1 (Adult)
* Mentor and assessing qualification (For newly qualified staff this is expected within 12 months of commencement)
Skills, Knowledge & Abilities
* Evidence of Continued Professional Development
* Must be able to demonstrate an understanding of confidentiality in all of the role
* IT literate (including Microsoft office) and willing to learn new systems
* Good communication skills both written and verbal with patients and across the MDT
* To be able to plan and deliver person centred care
Experience
* Working in a range of clinical settings
* Working in a community setting
