This is an exciting opportunity to join our existing team of Clinical Pharmacists working at Scott Arms Medical Centre on behalf of our PCN Health Vision Partnership.
The successful candidate will be part of a dynamic and forward-thinking team employed by Health Vision Partnership working at Scott Arms Medical Centre. Professional Leadership will be provided by Scott Arms Medical Centre Clinical Lead Partner.
The role is full time, 37.5 hours per week worked over 5 days.
Main duties of the job
The position of Primary Care Network Clinical Pharmacist has an essential role in improving the healthcare we provide to our patients. We are recruiting for a clinical pharmacist to work with existing practice teams to deliver consistency and excellence in medicines optimisation.
1. Improve patients' health outcomes and efficiency of the practice team by providing direct, accessible and timely medicines expertise, including identifying areas for improvement and initiating change.
2. Be the primary contact within the practice for queries relating to medicines management, in particular around individual patient care.
3. Focus on individual patient care by addressing immediate and future needs to personalise care planning.
4. Proactively transfer, from other clinical staff, workload relating to medicines optimisation to improve patient care and safety.
5. Demonstrate the effectiveness of the practice and the role through audit and data analysis.
6. Follow up patients and investigations, particularly blood tests.

Job responsibilities
The post holder is a clinical pharmacist, who acts within their professional boundaries, supporting and working alongside a team of Health Care Practitioners within general practice and a Primary Care Network (PCN).
The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will perform medicines reconciliation on transfer of care and implement systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the PCN.
The post holder will work as part of a multi-disciplinary team in a patient-facing role. They will take responsibility for areas of medicines optimisation within the PCN and undertake structured clinical medication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide clinical leadership on medicines optimisation and quality improvement (QI) and manage some aspects of the quality and outcomes framework and enhanced services at practice and PCN level.
Repeat prescribing
1. Develop and quality-assure improved, safe and efficient repeat prescribing and medication review processes within the practice, and then to evolve and implement changes as required.
2. Conduct focused and structured reviews of repeat prescriptions on an individual basis to improve quality and safety and maximise effectiveness and reduce wastage.
3. Provide medication review services via clinics in the practice, domiciliary visits and in residential and nursing homes.
Long-Term Conditions / Chronic Disease Management
1. Undertake disease-focused medication reviews, particularly of high-risk groups (elderly, renal/hepatic impairment), ensuring appropriate compliance with national and local guidelines.
2. Ensure the care for patients taking high-risk drugs e.g. DMARDs, anticoagulation, is safe and appropriate.
3. Provide telephone advice to patients with LTCs on specific medicines management issues.
4. Be familiar with, contribute to the development of, and work alongside practice ICB long-term conditions pathways to ensure consistency and efficiency of systems.
5. Provide health promotion where appropriate, including flu vaccinations.
Acute illness and pharmacist led clinics
1. Conduct initial assessments of common general practice presentations.
2. Work through differential diagnosis and identify serious disease and refer appropriately.
3. See patients with minor illnesses, independently diagnosing and managing the conditions.
4. Follow up with GP mentor support within boundaries of safety and competency.
5. Provide telephone advice to patients with acute illness queries.
6. Conduct minor ailment triage, with signposting and educating patients in the use of non-GP services.
Practice Formulary
1. Manage practice formularies and IT systems to ensure appropriate selection of medication is easy and maximised by prescribers in the practice.
Safety, Risk and Governance
1. Ensure appropriate governance is in place regarding medicines, including leading on the response to any drug safety alerts or changes in clinical guidance.
2. Assist with the development of relevant medicines management guidance and/or policies.
3. Meet regularly with the practice prescribing lead to discuss medication issues.
4. Advise the primary healthcare team on the safe and secure handling of controlled drugs and other medicines, ensuring compliance with medicines legislation.
5. Identify and flag up risks with regard to management of medicines including clinical and financial risks.
6. Offer advice and guidance on adverse significant events that involve medication to ensure lessons are learned and appropriate changes made to prevent re-occurrence.
7. Offer advice and guidance on patient complaints that relate to medication/prescribing.
Audit and Monitoring
1. Plan and undertake clinical/prescribing audits to identify areas where improvement is needed, develop action plans and implement appropriate changes.
2. Provide regular support and feedback on prescribing action plans.
3. Report regularly to the GP partners on the practice prescribing figures with benchmarking against other local and national data.
4. Analyse practice complex prescribing data to inform GPs and non-medical prescribers on their collective and individual performance.
5. Lead on the identification, implementation and monitoring of medicines management targets and initiatives including QOF, QIPP and CCG prescribing target.
6. Support the implementation of actions resulting from audit findings when necessary.
7. Agree all auditing and monitoring priorities in discussion with the practice.
Relationships
1. Provide training in medicines related issues to other members of the practice as appropriate.
2. Consider the skills of the reception team and develop plans for training as necessary to ensure safe and efficient prescribing processes.
Medicines Information and Awareness
1. Analyse and critically appraise complex information to prepare concise evidence-based summaries/advice to clinicians and nonclinical staff and patients.
2. Prepare evidence-based resources and information to support health professionals in the implementation of rational cost-effective prescribing.
3. Present at patient group meetings or other appropriate events to give advice on the appropriate use of medicines.
4. Produce patient information leaflets and posters, and run medicines awareness projects throughout the year.
5. Raise awareness of out of practice schemes, such as minor ailments and schemes that promote self-care.

Qualifications
* Mandatory registration with General Pharmaceutical Council (GPhC)
* May hold or be working towards an independent prescribing qualification.

* An appreciation of the nature of GPs and general practices.
* An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
