This is a fantastic opportunity to become a key member of the Tobacco and Alcohol Care Team as an adviser, where you’ll play a vital role in delivering assessments and providing targeted, impactful advice to patients across LTHTR. In this position, you’ll collaborate closely with a dedicated team of advisers and specialist nurses, ensuring that patients who smoke receive the highest standard of care.

Your role will be essential in enhancing the quality of care, while also contributing to the broader mission of tackling preventable illnesses, reducing health inequalities, and easing the demand on healthcare services.
You will be required to submit evidence of your qualifications for this position. If your qualifications were obtained overseas, please ensure that you acquire a Statement of Comparability from UK ENIC to facilitate a clear understanding of how your qualifications align with UK standards.
You will be responsible for managing a dedicated caseload, taking charge of coordinating and planning care for a specific group of patients to ensure their needs are thoroughly met. Your role involves providing specialised advice on specific conditions and treatment pathways, enabling the delivery of tailored care that suits each patient’s unique situation. A key focus will be on continuously improving patient care and contributing to the development of high-quality healthcare services.
In this role, you will assess patients holistically, planning, implementing, and evaluating evidence-based care plans that address all aspects of their well-being. Additionally, you will offer valuable advice and support to patients, their families, carers, and the wider multidisciplinary team, ensuring everyone involved is informed and empowered.
You will also play a crucial role in workforce development by delivering education, training, and expert advice, thereby helping to build a skilled and knowledgeable team. Your commitment to providing excellent care with compassion will ensure that every patient feels understood and valued. Moreover, you will work collaboratively with the multidisciplinary team (MDT) to ensure that patient management is cohesive, coordinated, and effective.
