Responsibilities:
1. Senior Social Worker to be based in both hospital and community team.
2. Facilitating discharges to an interim care home placement.
3. Managing and supporting a small team of social workers to facilitate discharge and provide a review and monitoring services to people in an interim care home placement.
Requirements:
1. Member of the SSSC
2. Social Work qualification
3. Excellent organisational skills and the ability to work flexibly, under direction and independently where necessary
4. Car driver
