Job summary To work as a member of the Urgent Community Response (UCR) squads, in the Rapid Response & Intermediate Care service, providing rapid interventions to support patients at home and avoid inappropriate admission into hospital. UCR squads are comprised of nurses, paramedics, physiotherapists, occupational therapists and healthcare assistants. The role will provide nursing, physiotherapy and occupational therapy based interventions for patients. It is a countywide role and part of the widerRapid Response & Intermediate Care service. The overall aims of the Rapid Response & Intermediate Care Team are to avoid inappropriate conveyances and admissions to hospital and facilitate timely discharge into the community. A full driving licence with business insurance and access to a car for work purposes is essential Main duties of the job Providing general and specific care to adults within competencies Being an integral member of the Urgent Community Response squad, providing defined delivery of care including core therapy, nursing and social rehabilitative care to patients in liaison with senior staff. Taking responsibility for designated elements of the patient treatment protocols and delivery of such treatments in patients' own homes. Being responsible for the completion and potential delegation of non-clinical tasks within the team. Improve the care and support of patients in the community, so patients can remain safely in their own home for as long as possible Have an innovative approach, excellent communication skills and willingness to work with other members of the community teams to ensure high quality service delivery Willingness to further develop your clinical expertise in a range of areas.
